This form is for TEACHERS and SCHOOL LIBRARIANS who would like to request materials to be picked up at the library. Public schools serving students who live in Matteson and private schools located in Matteson may pick up items at the library or have them delivered to their school. HOMESCHOOLERS may request items to be picked up at the library.

  • In most cases, this will be the due date of the assignment the items are to be used for. Please note that standard checkout time is 2 weeks, and we can not guarantee items will be available longer than that.
    Please select all the types of materials you are requesting. Resource kits include: Birds, Insects, People & Animals from Illinois' Past, Trees, & Nature Field Trip Backpack.
  • Include grade(s) of students using the materials. If more information is needed about reading levels, etc. please include it in the question below.
  • Please include any SPECIFIC TITLES, GENERAL TOPICS, or READING LEVELS you need covered in your material selection. Make sure to include the NUMBER OF COPIES of any specific item you need. Please provide the TOTAL NUMBER OF ITEMS YOU NEED.
